Safety and efficacy of Watchman left auricle closure implantation in patients with atrial fibrillation: a Meta-analysis on event singleton rate

Abstract:Objective To review systematically the safety and efficacy of Watchman left auricle closure implantation (Watchman occlusion) in patients with atrial fibrillation (AF) at home and abroad. Methods The databases of PubMed, EMbase, CNKI and WanFang Data were retrieved for searching domestic and overseas studies on Watchman occlusion for AF from Jan. 2000 to Jan. 2017. All literature was screened, extracted and reviewed on quality according to inclusion and exclusion criteria, and then reviewed systematically by using R software. Results There were totally 6 studies screened involved 916 patients. The results of Meta-analysis showed that incidence of total safety events of Watchman occlusion was 4.42% (95%CI: 1.30%~7.55%), including cardiac tamponade was 2.65% (95%CI: 0.11%~5.19%), intraoperative embolization was 1.09% (95%CI: 0.35%~1.82%), and operation-related bleeding was 0.09% (95%CI: 0.00%~0.39%). The incidence of totally effective events was 5.03% (95%CI:3.55%~6.52%), including stroke was 2.15% (95%CI: 0.00%~4.49%), thrombosis on device surface was 0.90% (95%CI: 0.26%~1.54%), and transient ischemic attack (TIA) was 0.01% (95%CI: 0.00%~0.30%). Conclusion Watchman left auricle closure implantation is safe and effective in AF patients with contraindications of oral anticoagulants.
