Influence of acute myocardial infarction accompanied by wide fragmented QRS wave on cardiac function

Abstract:Objective To investigate the influence of acute myocardial infarction (AMI) accompanied by fragmented QRS wave (fQRS) on cardiac function. Methods AMI patients (n=200) were chosen from the First Hospital of Handan City from Jan. 2016 to Nov. 2017. All patients were divided, according to whether or not they had fQRS in 12-lead electrocardiogram (ECG), into fQRS group and non-fQRS group. The indexes of ECG features and heart function were compared between fQRS group and non-fQRS group. And fQRS group was further divided, according to width of fQRS, into narrow fQRS group and wide fQRS group. The indexes of heart function, and incidence of arrhythmias and sudden cardiac death were compared between narrow fQRS group and wide fQRS group. The correlation among fQRS, heart function and prognosis in AMI patients was analyzed by using Pearson correlation coefficient analysis. Results In ECG feature indexes, QRS duration and QTc interval were all longer in fQRS group than those in non-fQRS group (P<0.05). The left ventricular ejection fraction (LVEF) was lower (P<0.05), and level of N-terminal pro-brain natriuretic peptide (NT-proBNP) was higher (P<0.05) in fQRS group than those in non-fQRS group. LVEF was lower (P<0.05), NT-proBNP level was higher (P<0.05), and incidence of arrhythmias and sudden cardiac death were higher (P<0.05) in wide fQRS group than those in narrow fQRS group. The results of correlation analysis showed that fQRS was negatively correlated to LVEF and prognosis, and positively correlated to NT-proBNP level, arrhythmias and sudden cardiac death in AMI patients. Conclusion The appearance of fQRS in ECG, especially wide fQRS, has adverse influence on heart function and will induce poor prognosis in AMI patients, so wide fQRS can be taken as an early-warning index for sudden cardiac death in clinical practice.
