Correlation between high-density lipoprotein-cholesterol (HDL-C) and severity of coronary artery stenosis

Abstract:Objective To study the relationship between high-density lipoprotein-cholesterol (HDL-C) and severity of coronary artery stenosis. Methods The patients (n=362, male 191, female 171, aged from 66 to 72 and average age=61.25±6.72) were chosen from the First People's Hospital of Xiangyang City of Hubei Province from Jan. 2015 to Sept. 2015. All patients were divided, according to results of coronary angiography (CAG), into normal group (n=113) and stenosis group (n=249). The stenosis group was divided, according to HDL-C level, into group Q1 (HDL-C>1.64 mmo1/L, n=57), group Q2 (HDL-C from 0.96 mmol/L to 1.51 mmol/L, n=63), group Q3 (HDL-C from 0.72 mmol/L to 0.91 mmol/L, n=68) and group Q4 (HDL-C<0.76 mmo1/L, n=61). The correlation between HDL-C level and severity of coronary artery stenosis was analyzed. Results The percentages of patients with hypertension, diabetes, smoking and drinking increased, levels of total cholesterol (TC), triglyceride (TG), low-density lipoprotein-cholesterol (LDL-C), free fatty acid (FFA) and Gensini integral increased, and HDL-C level decreased in stenosis group compared with normal group (all P<0.01). The difference in percentages of patients with hypertension, smoking and drinking, and levels of TC, TG, LDL-C, FFA and Gensini integral had statistical significance among groups Q1, Q2, Q3 and Q4 (all P<0.01). Gensini integral was positively correlated to TC, TG, LDL-C and FFA (P<0.01), and negatively correlated to HDL-C (r=-0.621, P<0.01) in patients with coronary artery stenosis. Multi-factor Logistic regression analysis showed that after corrected age, sex, body weight, height, diabetes, hypertension, heart rate, TC, TG, LDL-C and FFA, HDL-C was an independent protective factor of coronary artery stenosis (OR=0.949, 95%CI: 0.926-0.982, P<0.01). Conclusion HDL-C level will decrease significantly in patients with coronary artery stenosis, and it is significantly correlated to severity of coronary artery stenosis, and is an independent protective factor of coronary artery stenosis.
