Clinical features and characteristics of coronary artery lesions in elderly patients with coronary heart disease complicated by diabetes

Abstract:Objective To discuss the clinical features and characteristics of coronary artery lesions in elderly patients with coronary heart disease (CHD) complicated by diabetes.MethodsCHD patients (aged>65,n=250) were chosen from Jan. 2014 to June. 2017, and divided, according to whether or not with complicated diabetes, into diabetes group and non-diabetes group (eachn=125). The basic clinical materials and results of laboratory examination, echocardiogram and electrocardiogram (ECG) were collected within 24 h. The coronary artery lesions were divided, according to coronary stenosis≥50% involved left anterior descending branch (LAD) and left circumflex (LCX) of coronary artery, right coronary artery (RCA) and left main (LM) of coronary artery, into 1-vessel lesion group, 2-vessel lesion group (involved LM) and 3-vessel lesion group. The coronary artery lesion was scored by using Gensini score standard in each patient.ResultsThe level of fasting plasma glucose (FPG) increased significantly in diabetes group compared with non-diabetes group (P<0.001). The incidence of 3-vessel lesion was higher (P<0.001), and incidence of 1-vessel lesion and 2-vessel lesion were lower (P<0.001) in diabetes group than those in non-diabetes group. Compared with non-diabetes group, coronary artery lesions mostly involved LAD, LCX and RCA in diabetes group (P<0.05). The diseases vessels of coronary artery and Gensini scores were higher in diabetes group than those in non-diabetes group (P<0.001).Conclusion The lesions of coronary artery are more extensive and serious with higher deadly risk in patients with CHD complicated by diabetes.
