Diagnostic value of intravascular ultrasound to coronary artery lesions in patients with coronary heart disease

Abstract:Objective To discuss the diagnostic value of intravascular ultrasound to coronary artery stenosis and atherosclerosis plaque characteristics of in patients with coronary heart disease (CHD).Methods CHD patients (n=29, male 16, female 13, average age=60.6±7.7 and aged from 51 to 69) undergone PCI were chosen from the Central Hospital of Nanchong City from Jan. 2015 to Dec. 2016. All patients were given respectively coronary angiography (CAG) and intravascular ultrasound examination. The results of 2 detective methods were compared, including the diameter of most stenosis location of coronary artery, rate of diameter stenosis, atherosclerosis plaque characteristics and stenosis severity of diseased vessel segments.Results There were 29 patients (55 vessel segments) received CAG and intravascular ultrasound examination. The difference in results of stenosis severity at 55 vessel segments detected by 2 methods had no statistical significance (P>0.05). The detection rates of diameter of most stenosis location [(2.42±0.27) mmvs. (2.26±0.34) mm] and rate of diameter stenosis [(42.6± 7.4)%vs. (36.8±6.9)%] by intravascular ultrasound were higher than those by CAG (allP<0.05). There were 40 (72.73%) eccentric plaques detected by intravascular ultrasound and 29 (52.73%), by CAG (P<0.05). The natural determination results of 55 plaques by intravascular ultrasound showed that there were 31 soft plaques, 10 fibrous plaques, 8 calcified plaques and 6 mixed plaque.Conclusion The intravascular ultrasound examination is superior to CAG in detecting coronary stenosis and atherosclerosis plaque characteristics in CHD patients. It is of great value in guiding clinical interventional treatment.
