Relationship between severity of cerebral small vessel disease and dizziness

Abstract:Objective To study the relationship between severity of cerebral small vessel disease (CSVD) and dizziness in elderly population.Methods The patients (n=120) with white matter lesions (WML) were chosen according to the scale of age-related white matter changes (ARWMC) from the Department of Neurology of Chinese PLA Army General Hospital from Jan. 2017 to Sept. 2017. All patients were divided into dizziness group and non-dizziness group after history-taking, neurophysical examination, examination of head nuclear magnetic resonance and vestibular function test. The percentage of dizziness patients was counted, constituent ratio of CSVD severity and correlated between CSVD severity and symptoms of dizziness were analyzed, and relationship between damage of different brain regions and dizziness classification was analyzed in 2 groups.Results There was no significant difference in age, sex and educational background between 2 groups. The patients with chief complaints of dizziness accounted for 44.17%, and incidence of vertigo was 33.33%, dizziness, 28.33% and imbalance, 20.83%. The constituent ratio of CSVD severity had significant difference between 2 groups, and CSVD severity was correlated to symptoms of dizziness. Different types of dizziness involved in different brain regions, and vertigo was related mainly to frontal lobe, dizziness, mainly parietal-occipital lobe, and imbalance, mainly infratentorial region and frontal lobe.Conclusion The symptoms of dizziness are close related to CSVD in elderly population. Different types of dizziness can be observed in patients with CSVD, especially with severe WML.
