Application value of myocardial injury markers in early diagnosis of acute myocardial infarction

Abstract:Objective To analyze the application value of myocardial injury markers in early diagnosis of acute myocardial infarction (AMI). Methods The patients (n=122) with chest pain suspected with AMI were chosen from the CCU of Department of Cardiovascular Diseases in People's Hospital of Kaizhou District of Chongqing City from June 2016 to Feb. 2017, and among them 86 with diagnosed AMI were chosen as observation group and 36 without AMI, as control group. The level changes of serum creatine kinase-MB isoenzyme (CK-MB), cardiac troponin I (cTnI), myoglobin (Myo) and heart fatty acid binding protein (H-FABP) were detected at time points of within 3 h, 3 h-6 h and 6 h-12 h, and application value of all indexes in early diagnosis of AMI was discussed. Results The levels of H-FABP and Myo increased significantly in observation group [(51.37±15.78) vs. (0.31 ±0.12)] pg/ml compared with control group [(185.32±87.36) vs. (55.78±11.76)] ng/ml at time point of within 3 h (P<0.05), and maintained a higher levels at time points of within 3 h-6 h and 6 h-12 h (P<0.05). The levels of CK-MB and cTnI had no statistical difference between control group (t=1.9415, P=0.0545) and observation group (t=0.3961, P=0.6927) at time point of within 3 h, but increased at time points of within 3 h-6 h and 6 h-12 h (P<0.05). Conclusion The myocardial injury marker, H-FABP increases significantly 3 h before AMI attack, which is conducive to early detection of AMI and timely treatment. The level of Myo increases significantly within 3 h after AMI attack but with poor specificity, and it can be taken as a major auxiliary diagnostic indicator.
