A comparative study of the benefits and risks of new oral anticoagulants in elderly patients with nonvalvular atrial fibrillation

Abstract:Objective To study the benefits and risks of new oral anticoagulants (NOACs), dabigatran, and rivoroxaban in anticoagulation therapy for elderly patients with nonvalvular atrial fibrillation. Methods 55 elderly patients with nonvalvular atrial fibrillation were randomly divided into 2 groups. Among them, 27 patients were in the dabigatran group (dabigatran etexilate 110 mg 2/d orally); 68 patients were in the rivaroxaban group (rivaroxaban 10 mg 1/d orally). The changes of blood coagulation indexes and liver and kidney functions were measured before and after treatment, and the incidence of cardiovascular and cerebrovascular events and adverse drug reactions were observed at 12 months after treatment. Results The D-dimer in patients treated with dabigatran and rivaroxaban showed a downward trend. There was no significant difference between two groups with respect to the D-dimer after 12 months of treatment (P>0.05). The hemoglobin, liver and kidney function of the two groups did not change significantly before and after treatment (P>0.05). All patients treated with dabigatran and rivoroxaban did not have any adverse effect on heart rate, heart rhythm, cardiac structure and function before and after treatment. No cardiovascular or cerebrovascular event or bleeding event was observed in two groups at 12 months. Conclusions The study showed no difference between the two drugs in thromboembolic events, the incidence of major bleeding events and the rate of stroke events; and the benefits and risks were similar.
