Analysis of cardiac function and prognosis after implantation of CRT/CRTD pacemaker in patients with chronic heart failure

Abstract:Objective To study cardiac function and prognosis after implantation of CRT/CRTD pacemaker in patients with chronic heart failure (CHF). Methods 18 patients with CHF who were implanted CRT/CRT-D therapy were selected from April 2014 to April 2015. The patients were divided into two groups. Cardiac function and prognosis data, such as NYHA cardiac function grade, left ventricular ejection fraction (LVEF), left ventricular end-diastolic dimension (LVEDD), 6 min walking distance, pulmonary artery systolic pressure, LVEDD area, right ventricular size and other indicators were analyzed in these patients. Results After treatment, the symptoms of CHF in CRT/CRT-D patients were improved significantly. The QRS wave duration of left ventricular ECG was significantly shortened. The NYHA cardiac function grade was improved from (2.5±0.76) before operation to (3.7 ±0.65) after operation, while LVEDD regurgitation was reduced, and the final 6-minute walking distance was significantly increased from (235.51±108.45) m to (401.21±152.48) m. These results suggested that CRT/CRT-D implantation was effective on patients with CHF (P<0.05). The pulmonary artery systolic pressure, left atrium diameter and LVEDD area in CR/CRT-D patients were significantly improved after operation (P<0.05). The size of the right ventricle improved after operation, but the difference was not significant (P>0.05). There were no deaths in CHF patients implanted with CRT/CRTD for 12 months. The effective rate was 100.00%. There were only 1 case of left ventricular tachycardia and 1 case of diaphragmatic stimulation, all patients were implanted CRT, the total incidence of complications was 11.11%, patients implanted CRT-D during follow-up did not appear any complications. Conclusion CRT/CRT-D treatment of CHF patients can effectively improve cardiac function, and the effect is outstanding, with less adverse reactions.
