The predictive role of neutrophil-to-lymphocyte ratio for paroxysmal atrial fibrillation recurrence after radiofrequency ablation

Abstract:Objective To investigate the relationship between peripheral neutrophil-to-lymphocyte ratio (NLR) and recurrence after radiofrequency ablation in patients with paroxysmal atrial fibrillation (AF).Methods This study enrolled 127 paroxysmal AF patients who underwent radiofrequency ablation in Guoyao Dongfeng General Hospital of Hubei University of Medicine. The recurrence of AF was defined as detection of rapid atrial arrhythmias (≥30-second duration assessed by electrocardiogram or 24 h ambulatory electrocardiography) after 3 months of ablation. NLR values were calculated according to the result of preoperative routine blood test.Results The mean follow-up duration was (10.9±3.2) months, during which a total of 38 (29.9%) patients developed AF after radiofrequency ablation. The NLR value was significantly higher in patients with AF recurrence than without AF recurrence (3.57±0.71 vs. 2.61±0.36,P<0.01). Multivariate logistic regression result indicated that preoperative NLR can independently predict the recurrence of AF after ablation (OR=2.35, 95%CI: 1.70~3.28, P<0.01). ROC curve analysis result showed the area under the curve of NLR for AF recurrence prediction was 0.86 (95%CI: 0.79~0.93,P<0.01), with sensitivity of 72% and specificity of 86% at the optimal cut-off value of 3.42. Conclusion Preoperative NLR is closely associated with the recurrence of AF after radiofrequency ablation, and high NLR vale is an independent predictor for AF recurrence after ablation.
