Meta - analysis of the effect of hypertonic saline solution on patients with decompensated heart failure

Abstract:Objective To explore the effect of small volume hypertonic saline solution (HSS) on patients with decompensated chronic heart failure (CHF).Methods We electronically searched the following databases including Pubmed, Cochrane, Medline, EMbase, CBM, CNKI, VIP and WanFang Database to collect randomized controlled trials (RCTs) from inception to December, 2016. Two reviewers independently screened literature according to the inclusion and exclusion criteria, extracted data and assessed the quality of the included studies. Then, meta-analysis was Performed using RevMan 5.3 software.Results A total of 5 RCTs involving 2064 patients were included. Compared to HSS combined with diuretic, using diuretic alone had significantly higher mortality (RR=1.81, 95%CI: 1.53~2.15,P=0.23), and higher risk of re-admission (RR=1.99, 95%CI: 1.2~3.00,P=0.05).Conclusion Combined use of hypertonic saline solution can improve the mortality of CHF patients. The interpretation of this conclusion should be kept cautious. High quality studies with large sample size, rigorous design and long follow-up time are needed.
