Application value of color doppler flow imaging parameters in evaluation of approximal subclavian artery stenosis

Abstract:Objective To evaluate the value of Color Doppler Flow Imaging (CDFI) blood flow parameters in assessing proximal subclavian artery stenosis.Methods Fifty-two patients underwent cervical computer tomographic angiography (CTA) from March 2012 to June 2016 were retrospectively analyzed. All the CDFI parameters including EDV, PSV1, PSV1/PSV0, PSV1/PSV2 were recorded. The Kappa values were calculated using SPSS software to evaluate the consistency between CDFI blood flow parameters and carotid artery CTA results, and ROC curve was calculated to evaluate the diagnostic value of CDFI blood flow parameters.Results 35 positive cases and 17 negative cases were diagnosed by CDFI, and 38 positive cases and 14 negative cases were diagnosed by CTA. There were 3 false-positive cases and 4 false-negative cases in CDFI diagnosis. The diagnostic sensitivity, specificity and Kappa value of CDFI were 89%, 79% and 0.665 respectively. The diagnostic value of PSV1/PSV0 and PSV1/PSV2 was the highest, and the area under the ROC curve was> 0.90. When PSV1/PSV0=1.96 was the diagnostic threshold, the sensitivity and specificity of diagnosis stenosis were 94% and 87% respectively. When PSV1/PSV2=3.34 was the diagnostic threshold, the sensitivity and specificity of diagnosis stenosis were 83% and 100%, respectively.Conclusion CTA and CDFI results are moderately consistent. Ratio between stenostic velocity and contralateral velocity(PSV1/PSV0) and ratio between stenostic velocity and distal post-stenostic velocity (PSV1/PSV2) are the most valuable parameter for the diagnosis.
