Meta-analysis of dual antiplatelet therapy 12 months and>12 months after percutaneous coronary intervention

Abstract:Objective To systematically review the duration of dual antiplatelet therapy after percutaneous coronary intervention.Methods Database including PubMed, The Cochrane Library, EMbase, Web of Science, CBM, WanFang Data, VIP, CNKI were searched from inception to Nov.2016, to collect RCTs about duration of dual antiplatelet therapy after percutaneous coronary intervention. Meta-analysis was performed by RevMan 5.3 software. Results A total of 12 RCTs including 41728 patients. The ≤12 months' group has 20768 patients and the >12 months' group has 20960 patients. The result of meta-analysis showed: the ≤12 months' group significantly reduce the risk of total mortality (RR=0.85, 95%CI: 0.74 to 0.97,P=0.02) and the risk of major bleeding (RR=0.61, 95%CI: 0.42 to 0.90,P=0.01), but significantly increase the risk of stent thrombosis (RR=1.78, 95%CI: 1.19 to 2.67, P=0.005), and the rate of myocardial infarction (RR=1.68, 95%CI: 1.48 to 1.91,P<0.00001). There is no significant difference between the two groups in cardiac death, stroke, TVR, minor bleeding.Conclusion Shorter duration of dual antiplatelet therapy after percutaneous coronary intervention can reduce the total mortality, major bleeding and increase the stent thrombosis, myocardial infarction rather than cardiac death, stroke, TVR, minor bleeding.
