Predictive value of intracoronary electrocardiogram to short-term prognosis in patients with acute myocardial infarction

Abstract:Objective To analyze the predictive value of intracoronary electrocardiogram (IC-ECG) to short-term prognosis in patients with acute myocardial infarction (AMI).Methods The patients with the first attack of AMI (n=29, male 22, female 7 and average age=54.8±12.8) were chosen from Xuanwu Hospital from May 2012 to Jan. 2013. The patients were divided into group A (n=21, 50% ST-segment resolution time<40 s) and group B (n=8, 50% ST-segment resolution time>40 s). All patients were given percutaneous coronary intervention (PCI). The level of left ventricular ejection fraction (LVEF) was detected after AMI for 1 wk. The patients were given detection of 99mTc-MIBI myocardial perfusion tomography imaging (MPTI), and after 24 h were given again nitroglycerin-mediated myocardial perfusion tomography imaging. The pixel value of defect area and percentage of radiocounting in detect area were compared between 2 groups. The incidence of post-infarction angina pectoris, ventricular arrhythmias (VA), congestive heart failure (CHF) and death were recorded after PCI for 4 wk.Results LVEF was (56.7±6.6)% in group A and (48.8±5.9)% in group B after AMI for 1 wk (P<0.05). The rest pixel value of defect area was (220±43.4) in group A and (241±48.6) in group B, and after nitroglycerin mediated, pixel value of defect area was significantly decreased in group A compared with group B (104.8±31.6)vs. (209±33,P<0.05). The percentage of rest radiocounting in detect area was (24.2±7.1)% in group A and (20.4±6.5)% in group B. The rest pixel value of defect area was significantly higher in group A than that in group B (29.9±5.7)%vs. (23.2±5.5)%after nitroglycerin mediated (P<0.05). After PCI for 4 wk, there were 4 (19.0%) cases of post-infarction angina pectoris, 4 (19.0%) of VA, and 2 (9.5%) of CHF in group A, and there was 1 (12.5%) case of post-infarction angina pectoris and 1 (12.5%) of VA, and there were 3 (37.5%) of CHF in group B. There were no cases died in 2 groups. The incidence of CHF was higher in group B than that in group A (P<0.05).Conclusion The time of ST-segment resolution of IC-ECG is short and short-term prognosis is good in AMI patients.
