Influence of different levels of systolic blood pressure on new-onset stroke in non-hypertension population

Abstract:Objective To analyze the influence of different levels of systolic blood pressure (SBP) on new-onset stroke in non-hypertension population.Methods The prospective cohort study method was applied to extract relevant data from the first physical examination from Jul. 2006 to Oct. 2007, and the second, third and fourth physical examinations from 2008 to 2009, 2010 to 2011 and 2012 to 2013 in the on-the-job and retired staff of Kailuan Group. The first physical examination was taken as starting point of follow-up, and new-onset stroke, as outcome event. The end time of follow-up was Dec. 31, 2013. All staff was divided, according to baseline SBP level, into group A (SBP<120 mmHg,n=25239), group B (120 mmHg≤SBP<130 mmHg,n=19087) and group C (130≤SBP<140 mmHg,n=10433).Results There were totally 54759 cases chosen including 41321 male cases and 13438 female cases, and baseline average age was (48.48±12.53). Age, blood pressure, high-density lipoprotein-cholesterol (HDL-C), fasting blood glucose (FBG) and body mass index (BMI) showed an ascending trend in group A, group B and group C (allP<0.05). The follow-up time was averagely 6.97 y. There were 825 patients with new-onset stroke, and incidence of stroke was 1.06% in group A, 1.67% in group B and 2.29% in group C showed an ascending trend (allP<0.05). The incidence of stroke in male cases was 1.20% in group A, 1.85% in group B and 2.53% in group C showed an ascending trend (allP<0.05), and that in female cases was 0.76% in group A, 0.94% in group B and 1.10% in group C. The results of multivariate Cox proportional hazards regression model showed that the stroke risk was 1.26 times in group C (95%CI: 1.05-1.51) and 1.24 times in group B (95%CI: 1.05-1.46) higher than that in group A (allP<0.05) after revising age, sex, BMI, total cholesterol, triglyceride, smoking, drinking and education background. The stroke risk in male cases was 1.41 times (95%CI: 1.15-1.72) in group C and 1.33 times (95%CI: 1.11-1.60) in group B (allP<0.05).Conclusion The incidence and risk of new-onset stroke will increase as SBP increasing in male non-hypertension population.
