The effects of strength statins in the prevention of contrast induced nephropathy after percutaneous coronary intervention or coronary angiography in the elder patients: a meta-analysis

Abstract:Objective To evaluate the effectiveness of statin therapy in the prevention of contrast-induced nephropathy (CIN) in elderly patients after percutaneous coronary intervention.Methods The Cochrane library, PubMed, Embase, Web of Science, CNKI, Wanfang Data, VIP and CBM were searched for randomized controlled trials (RCTs) about the use of statins to prevent the occurrence of CIN in elderly patients after PCI from inception to February 2015. RCT screening, data extraction and quality evaluation were performed independently by three investigators according to the inclusion exclusion criteria. Meta-analysis was performed using RevMan 5.2 software. Results A total of 13 RCTs and 2722 patients were included. Meta-analysis showed a significant reduction in the incidence of CIN after intensive statin therapy among elder patients (RR=0.41, 95%CI: 0.32~0.53,P<0.00001). Subgroup analysis showed that intensive statin therapy significantly improved the incidence of CIN in patients with diabetes mellitus (RR=0.38,P=0.002) and renal insufficiency (RR=0.44,P<0.00001), the level of BUN decreased (MD=-0.80, 95%CI: -1.28~0.32,P<0.0001), the level of Scr decreased (MD=-8.10, 95%CI: -11.06 ~ -5.13, P <0.00001) (MD= -0.15, 95%CI: -0.19~-0.11,P<0.00001), the level of CysC decreased (MD=5.88, 95%CI:2.67-9.09,P=0.0003), hs-CRP increased (MD=-2.66, 95%CI:-3.46~1.86,P<0.00001).Conclusion Intensive statin therapy can reduce the incidence of CIN in elderly patients after percutaneous coronary intervention, protect renal function, and significantly reduce the inflammatory response. The role of rosuvastatin and atorvastatin in the prevention of CIN incidence in elderly patients is similar.
