Relationship between J-curve of systolic blood pressure (SBP) and major adverse cardiovascular and cerebrovascular events in patients with senile coronary heart disease complicating by hypertensive

Abstract:Objective To investigate the relationship among J-curve, systolic blood pressure level during systolic blood pressure decreasing and major adverse cardiovascular and cerebrovascular events (MACCE) in patients with senile coronary heart disease (CHD) complicating by hypertensive.Methods The patients with CHD complicating by hypertensive (n=323) were chosen from the Department of Cardiology of Central Hospital of Tongchuan Mining Bureau from Mar. 2010 to Mar. 2012. The patients were divided, according to SBP level, into 5 groups: group 1 (n=55, SBP<120 mmHg, group 2 (n=64, SBP 120 mmHg~129 mmHg), group 3 (n=79, SBP 130 mmHg~139 mmHg), group 4 (n=67, SBP 140 mmHg~149 mmHg) and group 5 (n=58, SBP≥150 mmHg). The incidence of MACCE was compared among 5 groups. A Cox regression model was established and curve graph was drawn for tendency analysis.Results There were 198 patients with MACCE during follow-up period. The Cox regression analysis showed that the incidence of MACCE increased significantly when SBP was higher than 140 mmHg after regulating age, body mass index (BMI) and SBP. When SBP was controlled from 120 mmHg to 130 mmHg, the risk of MACCE was the lowest, and when SBP<120 mmHg, the incidence of MACCE, however, increased slightly.Conclusion J-curve phenomenon exists during hypotension process in elderly patients with CHD complicated by hypertension, so blood pressure should be reasonably controlled for reducing the incidence of MACCE.
