Correlation between blood pressure and glycated hemoglobin in elderly male patients with versa-dipper hypertension or non-dipper hypertension

Abstract:Objective To discuss the correlation between glycated hemoglobin (HbAlc) and versa-dipper hypertension in elderly male patients.Methods The elderly male patients with primary hypertension (n=650, average age=79.49± 10.42) were chosen from the Department of Cardiology of Chinese PLA Hospital in Beijing from Jan. 2009 to Dec. 2012. All patients were randomly divided into non-dipper hypertension group (group 1,n=369) and versa-dipper hypertension group (group 2,n=281). HbAlc was given quarterback grouping. The general clinical data was collected from all patients, who were given relevant biochemical tests, and examinations of cardiac ultrasonography and ambulatory electrocardiograph monitoring (Holter).Results The age and percentages of patients complicated by coronary heart disease and carotid atherosclerosis were higher in group 2 than those in group 1. The levels of 24-h systolic blood pressure [SBP, (126.26±11.94) mmHgvs. (128.78 ±12.33) mmHg], nighttime SBP [(120.10±12.95) mmHgvs. (135.10±13.84) mmHg] and nighttime diastolic blood pressure [DBP, (61.76±9.05) mmHgvs. (66.76±8.35) mmHg] were higher, and daytime SBP [(128.79±12.04) mmHgvs. (126.18± 12.25) mmHg] and daytime DBP [(67.33±9.46) mmHgvs. (64.17±7.62) mmHg] were lower in group 2 than those in group 1. The outcomes of cardiac ultrasonography showed that interventricular septal thickness [(10.42±1.19) mmvs. (10.63±1.33) mm], left ventricular posterior wall thickness [(10.10±0.81) mmvs. (10.28±0.95) mm] and left ventricular end-diastolic volume [(111.90±21.06) mLvs. (115.57±19.78) mL] were higher, and left ventricular ejection fraction [(61.90±4.64)%vs. (60.67±5.62)%] was lower in group 2 than those in group 1 (P<0.05). According to HbAlc quarterback grouping, as HbAlc level increased, the percentage of patients with versa-dipper hypertension increased (P<0.05). The multi-factor regression analysis showed that HbAlc level was negatively correlated to the mean of nighttime DBP (r=-0.067) after correcting age, medical history and relevant tests.Conclusion The disappearance of circadian rhythm of blood pressure is more significance and injury of target organs was more serious in patients with versa-dipper hypertension. HbAlc level may play some roles in occurrence and development of changes of circadian rhythm of blood pressure.
