Monitoring value of D-dimer (D-D) level in treatment of deep vein thrombosis of lower extremityMA

Abstract：Objective To investigate the guidance value of D-dimer (D-D) level in treatment of deep vein thrombosis of lower extremity.Methods The patients with deep vein thrombosis of lower extremity (n=80) were chosen from the Cangzhou Hospital of Traditional Chinese Medicine and Western Medicine from Sept. 2014 to Jan. 2016, and then divided randomly into test group and control group (eachn=40). The level of plasma D-D was dynamically monitored by using enzyme-linked immunosorbent assay (ELISA) for adjusting the doses of low molecular weight heparin and warfarin, and maintaining D-D level from 0.5 mg to 1.0 mg in test group. The doses of low molecular weight heparin and warfarin were adjusted according to international normalized ratio (INR) for maintaining INR from 2.0 to 2.5 in control group. The circumference of lower extremity, diameter stenosis centigrade (STEN) of occluded vein, plasma D-D level and INR value were compared in 2 groups at different time points before and after treatment.Results The difference value of circumference of lower extremity between lesion side and healthy side was lower in test group than that in control group after treatment for 14 d (P<0.05). STEN of occluded vein was lower in test group than that in control group after treatment for 3 d and 14 d (P<0.05). D-D value was higher in test group than that in control group after treatment for 3 d (P<0.05). D-D value was lower in test group than that in control group after treatment for 14 d (P<0.05). The incidence of complications was lower in test group (0%) than that in control group (10.00%,P=0.040). After treatment for 3 m, the scores of quality of life (QOL) were higher in test group than that in control group (P<0.05).Conclusion The adjusted dose of warfarin according to monitored D-D level can improve thrombolysis effect and ensure safety in treatment of deep vein thrombosis of lower extremity.
