Effect of valsartan combined with amlodipine on BPV in essential hypertension patients and its mechanism

Abstract：Objective To investigate the effect of valsartan combined with amlodipine on the antihypertensive effect of blood pressure variability in patients with essential hypertension and its mechanism.Methods 140 patients with essential hypertension treated by cardiovascular medical treatment from January 2010 to December 2015 in Haimen Fourth People's Hospital of Jiangsu Province were selected. The patients were randomly divided into experimental and control groups. The 70 patients in the trial group were treated with valsartan and amlodipine and the control group was treated with valsartan and hydrochlorothiazide. Blood pressure control, blood pressure variability, plasma nitric oxide (NO), endothelin (ET) levels were compared between the two groups after 6 and 12 weeks of treatment, respectively.Results Before treatment, after 6 weeks and 12 weeks of treatment, there was no statistical significance between two groups among 24 h mean systolic pressure (24 hSBP), 24 h mean diastolic blood pressure (24 hDBP), daytime systolic blood pressure (dSBP), day diastolic blood pressure (dDBP), nighttime systolic blood pressure (nSBP), night diastolic blood pressure (nDBP) (P>0.05). The 24 h systolic blood pressure variability (24 hSBPV), 24 h diastolic blood pressure variability (24 hDBPV) of the experimental group were significantly lower than those of the control group (P<0.05) after 6 and 12 weeks of treatment. ET in experimental group was lower than that in control group, while NO was higher than that in control group, the difference was significant (P<0.05). There was no significant difference between the two groups with respect to blood pressure compliance rate (84.29%vs. 80.00%,P>0.05).Conclusion The efficacy of valsartan combined with amlodipine in treatment of essential hypertension was similar to that of valsartan combined with hydrochlorothiazide, but the effect of valsartan combined with amlodipine on blood pressure variability was better than that of amlodipine combined with amlodipine. The mechanism may be related to the improvement of vascular endothelial function.
