Curative effect and safety of Bosentan in treatment of pulmonary arterial hypertension

Abstract:Objective To observe the curative effect and safety of Bosentan in treatment of congenital heart disease-related pulmonary arterial hypertension (CHD-PAH) and idiopathic pulmonary arterial hypertension (IPAH).Methods The patients with CHD-PAH or IPAH (n=83) were chosen from the Department of Cardiovascular Diseases of Mudan People’s Hospital of Heze City in Shandong Province from Feb. 2014 to Mar. 2016, divided into CHD-PAH group (n=48) and IPAH group (n=35) and were treated with Bosentan. The indexes of cardiac function grading of PAH, hydropericardium, 6-minute walk distance (6MWD), color Doppler echocardiogram, right cardiac catheterization examination and MOS 36-item Short Form Health Survey (SF-36) were compared between 2 groups before and after treatment.Results After Bosentan treatment for 16 w, except of aortic root diameter (AO) and social function score, cardiac function grading of PAH, hydropericardium, 6MWD, left atrial diameter (LAD), left ventricular end-diastolic inner diameter (LVEDd), left ventricular end-systolic diameter (LVESd), interventricular septal thickness (IVST), systolic pulmonary artery pressure (SPAP), mean pulmonary arterial pressure (mPAP), right atrial pressure (RAP), pulmonary vascular resistance (PVR), cardiac index (CI), pulmonary capillary wedge pressure (PCWP) and other 7 items scores in addition to social function score in SF-36 were all improved in 2 groups. The indexes of 6MWD, LAD, SPAP, mPAP, RAP, PVR, CI and 7 items scores in addition to social function score in SF-36 were all improved significantly in 2 groups after treatment (P<0.05). The comparison in LAD, LVEDd and LVESd had statistical difference (P<0.05), and comparison in other indexes had no statistical difference (P>0.05) between 2 groups after treatment. The indexes of blood routine examinations and relevant biochemical indicators had no significant changes, and 2 patients in each group had mild transaminase increases during treatment and they were recovered after liver protection treatment for 1 w.Conclusion Bosentan has curative effect on both CHD-PAH and IPAH, can improve significantly patients’ quality of life and is relatively safe.
