Value of dual-energy CT myocardial perfusion imaging in diagnosis of coronary heart disease

Abstract:Objective To discuss the value of dual-energy CT (DECT) myocardial perfusion imaging (MPI) in diagnosis of coronary heart disease (CHD).Methods The patients with diagnosed CHD or suspected CHD were chosen as observation group and the subjects with physical examinations were chosen as control group (each n=72) from the Department of Cardiology of Hai’an People’s Hospital affiliated to Nantong University from Dec. 2013 to Jan. 2016. All groups were given routine CT and DECT-MPI examinations, and at the same time the observation group was given coronary angiography (CAG) examination for correlation analysis.Results The CT images quality of all cases met the diagnostic requirement. Of 72 patients in observation group, there were 65 (135 coronary branches) with stenosis or occlusion in varying degrees, and there were no cases with stenosis in control group. In observation group, diastolic CT value was (45.25±16.22) HU and systolic CT value was (44.29± 12.64) HU, which were significantly lower than those in control group [(107.39±18.98) HU, (101.48±18.22) HU, P<0.05]. Among 72 patients in observation group, there were 62 diagnosed with CHD, and 40 with mild stenosis, 20 with moderate stenosis and 2 with severe stenosis. In the diagnosis of CHD, the sensitivity of DECT-MPI was 95.4% and specificity was 100%. Spearman correlation analysis showed that diastolic CT value and systolic CT value of DECT-MPI were negatively correlated to stenosis degree of coronary artery (r=-0.563, -0.622,P<0.05). Conclusion DECT-MPI can obtain excellent images of coronary artery and myocardial perfusion with higher specificity and sensitivity, which can reflect disease status and has higher diagnostic values in clinic.
