Curative effect of levosimendan combining recombinant human brain natriuretic peptide in patients with acute decompensated heart failure

Abstract:Objective To observe the curative effect of levosimendan combination recombinant human brain natriuretic peptide (rhBNP) in patients with acute decompensated heart failure (ADHF).Methods ADHF patients (n=90, male 52, female 38, aged from 41 to 78 and average=61.5±19.4) were chosen from the Department of Cardiology of People’s Hospital in Baoji City of Shaanxi Province from Jan. 2012 to Jan. 2014, and randomly divided into combination group, levosimendan group and rh-BNP group (eachn=30). The heart rate (HR), respiratory rate (RR), systolic blood pressure (SBP), oxygen saturation (SpO2), 24-h urine volume, serum creatinine (SCr) and glomerular filtration rate (GFR) were recorded before treatment and at the time points of 6 h, 12 h, 24 h, 48 h and 72 h after treatment. The changes of left ventricular end-diastolic inner diameter (LVEDd), left ventricular ejection fraction (LVEF) and brain natriuretic peptide (BNP) were detected in 2 groups before and 72 h after treatment. The incidence of adverse reactions (allergic reaction and gastrointestinal reactions, etc.) was observed during treatment period.Results The levels of RR and SCr decreased significantly, and SpO2, GFR and 24-h urine volume increased in 3 groups at the time points of 6 h, 12 h, 24 h, 48 h and 72 h after treatment (all P<0.05). Compared with rh-BNP group and levosimendan group, RR decreased and 24-h urine volume increased in combination group at the time points of 6 h, 12 h, 24 h, 48 h and 72 h after treatment (allP<0.05). LVEDd and LVEF were improved in 3 groups 72 h after treatment (allP<0.05), which was more significant in combination group compared with other 2 groups (allP<0.05). The level of BNP decreased significantly in 3 groups after treatment, which was more significant in combination group compared with other 2 groups (allP<0.05) at the same time. During treatment period, the indexes of liver function, electrolytes and coagulation had no significant changes and there were no cases of gastrointestinal reactions and allergic reaction related to drugs.Conclusion Levosimendan combining rhBNP can significantly relieve clinical symptoms and improve heart function in ADHF patients and it is safe and reliable.
