Clinical significance of early ST-segment changes in patients with ST-segment elevation myocardial infarction after percutaneous coronary intervention

Abstract:Objective To investigate the clinical significance of ST-segment reduction in patients with acute ST-segment elevation myocardial infarction (STEMI) after emergent percutaneous coronary intervention (PCI). Methods STEMI patients (n=150) were chosen from Jan. 2011 to Sept. 2013 and given retrospective analysis. All patient were divided, according to whether or not percentage of ST-segment reduction>50%, into group A (n=84, percentage of ST-segment reduction>50%) and group B (n=66, percentage of ST-segment reduction≤50%) within 1 h after PCI. The myocardial enzymes indexes before and 24 h after PCI, indexes of heart function 2 w after PCI and incidence of major adverse cardiovascular events (MACE) were compared in 2 groups.Results The levels of creatine kinase (CK), creatine kinase MB (CK-MB) and cardiac troponin T (cTnT) were significantly lower in group A than those in group B before and 24 h after PCI (P<0.05). The difference in left ventricular ejection fraction (LVEF) had no statistical significance between 2 groups before PCI (P>0.05), and LVEF was significantly higher in group A than that in group B 24 h after PCI (P<0.05). The left ventricular end-diastolic volume (LVEDV) was significantly lower in group A than that in group B before and 24 h after PCI (P<0.05). The incidence of MACE was 9.52% in group A and 31.82% in group B (P<0.05) 1 y after PCI.Conclusion The heart function and myocardial enzyme indexes will be recovered better and long-term incidence of MACE will be lower in STEMI patients with rapid ST-segment reduction (percentage of ST-segment reduction>50%) after emergent PCI.
