Characteristics of heart rate variability and intervention effect of metoprolol in elderly patients with coronary heart disease

Abstract:Objective To investigate characteristics of heart rate variability (HRV) and the clinical effect of using metoprolol in elderly patients with coronary heart disease (CHD).Method 70 elderly CHD patients of PLA Army General Hospital from March 2015 to February 2016 were selected and divided into metoprolol group (n=45) and non- metoprolol group (n=25) depending on whether administrated metoprolol or not. 32 patients without CHD of same period were enrolled as non-CHD group. Continuously recording electrocardiograph (ECG) for 24 hours were conducted using 24h dynamic electrocardiogram and the corresponding analysis system. Indexes of heart rate variability including standard deviation of normal to normal (SDNN), standard deviation of the averages of normal to normal (NN) intervals in all 5 min segments of the entire recording (SDANN), mean of the standard deviations of all NN intervals for all 5 min segments of the entire recording (SDNN Index), The square root of the mean of the sum of the squares of differences between adjacent NN intervals (rMSSD), NN50 count divided by the total number of all NN intervals. (pNN50), total power (TP), power in very low frequency range (VLF), power in low frequency range (LF) and power in high frequency range (HF) were analyzed, and heart rate variability was compared in different groups.Results Average SDNN and SDANN in CHD patients were significantly lower than non-CHD group (P<0.05). There was no significant difference with respect to other indexes of time domain and frequency domain between CHD patients and non-CHD group (P>0.05). Average LF and HF in metoprolol group were significantly lower than non- metoprolol group (P<0.05). There was no significant difference with respect to other indexes of time domain and frequency domain between metoprolol group and non- metoprolol group (P>0.05).Conclusion The HRV in elderly CHD patients is decreased. Metoprolol is helpful to adjust the autonomic nervous function of patients and to improve the HRV.
