Influencing factors of myocardial injury in patients with viral hepatitis

Abstract:Objective To analyze the severity of myocardial injury and relevant influencing factors in patients with viral hepatitis.Methods The patients (n=120, male 68, female 52 and aged from 20~64) were chosen from Medical Treatment Center of Wuhan City from May 2013 to May 2015. The patients were divided, according to etiology, into hepatitis A group (n=15), hepatitis B group (n=85), hepatitis C group (n=12) and hepatitis D group (n=8). And all patients were divided again, according to age, into <50 group (n=38), 50~59 group (n=50) and>60 group (n=32). The indexes of myocardial injury including creatine kinase (CK), lactic dehydrogenase (LDH), cardiac troponin I (cTnI), myoglobin (Mb) and creatine kinase MB (CK-MB), and indexes of liver function including albumin, total bilirubin (TBIL), total bile acid (TBA), alanine aminotransferase (ALT) and aspertate aminotransferase (AST) were detected.Results The levels of CK, LDH, MB and CK-MB were higher in hepatitis B group than those in hepatitis A group, hepatitis C group and hepatitis D group (allP<0.05). The levels of cTnI and MYO were higher in hepatitis C group than those in hepatitis A group, hepatitis B group and hepatitis D group (allP<0.05). The level of albumin decreased, and levels of TBil, TBA, ALT and AST increased significantly in hepatitis C group compared with hepatitis A group, hepatitis B group and hepatitis D group (allP<0.05). The levels of TBA, ALT and AST showed a ascending trend in hepatitis A group, hepatitis D group, hepatitis B group and hepatitis C group (allP<0.05). The levels of CK, LDH, cTnI, MB, MYO and CK-MB were all higher in >60 group than those in 50~59 group and <50 group, and were higher in 50~59 group than those in <50 group (allP<0.05). The results of Logistic regression analysis showed that age>60 (OR=5.238,95%CI:2.260~15.263), hepatitis course≥10 years (OR=3.562,95%CI:1.075~8.279), hepatitis B (OR=4.415,95%CI:1.255~2.135) and abnormal liver function (OR=3.556,95%CI:1.024~6.239) are risk factors of myocardial injury in patients with viral hepatitis. Conclusion Liver function and myocardial injury severity are very different in patients with different types of viral hepatitis. Age>60, hepatitis course≥10 years, hepatitis B and abnormal liver function are risk factors of myocardial injury, and controllable factors should be intervened during treatment.
