The predictive value of ST segment changes in lead aVR for patients with NSTE-ACS

Abstract:Objective To investigate predictive value of ST segment changes in lead aVR for patients with non-ST elevation acute coronary ischemic syndrome (NSTE-ACS).Methods 81 NSTE-ACS patients from October 2013 to December 2015 in Huai'an First People's Hospital were selected, including 24 patients with ST segment elevation in lead aVR, 57 patients non-ST segment elevation in lead aVR. Differences of cardiac function indexes and coronary angiography results in patients with or without ST segment elevation in aVR lead were compared. Predictive ability of different ST segment elevation in aVR lead for patients with left main lesion combined multiple vessel lesion.Results The Systolic blood pressure, diastolic blood pressure, heart rate, cardiac troponin I in patients with ST segment elevation in aVR lead were higher than patients without ST segment elevation in aVR lead (P<0.05). The left ventricular ejection fraction in patients with ST segment elevation in aVR lead were lower than patients without ST segment elevation in aVR lead (P<0.05). Left main lesion (29.17%), multiple vessel lesions (58.33%), left main lesion combined multiple vessel lesion (66.67%) in patients with ST segment elevation in aVR lead were higher than patients without ST segment elevation in aVR lead (5.26%, 12.28%, 15.79%,P<0.05). The prediction sensitivity, specificity and accuracy of different grade of ST segment elevation in aVR lead for left main lesion combined multi vessel lesions were (84.0%、87.50%、86.42%), (52.0%、92.86%、80.25%) and (20.0%、98.21%、74.07%), respectively.Conclusion ST segment elevation in aVR lead was important for prediction of left main lesion combined multiple vessel lesion in NSTE-ACS patients.
