Clinical efficacy of insulin detemir combining acarbose in elderly patients with incipient type 2 diabetes mellitus

Abstract:Objective To observe the curative effect and safety of insulin detemir combining acarbose in elderly patients with incipient type 2 diabetes mellitus (T2DM).Methods T2DM patients (n=58, male 41, female 17, aged from 61 to 74 and average age=66±5.5) were chosen from Nov. 2013 to Jan. 2015, and then treated with insulin detemir combining acarbose for 12 w. The changes of fasting blood glucose (FBG), 2-hour postprandial blood glucose (2-hPBG), glycated hemoglobin (HbAlc), total cholesterol (TC), triglyceride (TG), low-density lipoprotein-cholesterol (LDL-C) and high-density lipoprotein-cholesterol (HDL-C) were observed before and 12 w after treatment, and incidence of hypoglycemia and adverse reactions were detected during treatment.Results After treatment for 12 w, the levels of FBG and HbA1c decreased significantly in all patients [(6.8±0.8) mmol/Lvs. (10.7±1.2) mmol/L]、[(7.5±1.2)%vs. (8.8±1.7)%, allP<0.05]. The comparison in all indexes of blood fat and body mass index showed no statistical difference before and after treatment (allP<0.05). The incidence of daytime hypoglycemia was 1.72%, and there were no nighttime semiotic hypoglycemia and severe hypoglycemia observed during treatment.Conclusion Insulin detemir combining acarbose can effectively reduce blood sugar level and does not increase body weight in elderly patients with incipient T2DM. The incidence of hypoglycemia is lower.
