Risk Factors for Contrast-induced Nephropathy after coronary angiography in patients with acute coronary syndrome

Abstract:Objective To compare the incidence and risk factors of contrast-induced nephropathy (CIN) after coronary angiography (CAG) in Patients with acute coronary syndrome (ACS).Methods 119 patients with CIN and 1201 patients without CIN were selected as CIN group and control group from January 2014 to January 2015, and their clinical data were analyzed.Results Patients in CIN group were older, and number of patients combined with diabetes mellitus (DM), chronic renal insufficiency, anemia and hypotension was higher than control group. The value of White blood cell count (WBC), blood urea nitrogen (BUN), serum creatinine (SCr), blood glucose Glu), troponin T (TNT), brain natriuretic peptide (BNP), intraoperative contrast medium (CM) were higher in CIN group (P<0.05); but left ventricular ejection fraction (LVEF) in CIN group is lower than control group (P<0.05). Result of logistic regression analysis showed that over 70 years old of age, history of chronic kidney disease (CKD), over 200ml of CM, less than 100 mmhg of systolic blood pressure at admission, LVEF<50%, WBC>10×109/L, TNT>0.5 ng/ml, BNP>300 ng/ml, were independent risk factors of CIN after CAG.Conclusion over 70 years old of age, history of chronic kidney disease (CKD), over 200ml of CM, less than 100 mmhg of systolic blood pressure at admission, LVEF<50%, WBC>10×109/L, TNT>0.5 ng/ml, BNP>300 ng/ml, were independent risk factors of CIN after CAG in ACS patients.
