Lactic acid and saturated hydrogen saline could attenuate myocardial apoptosis by mimicking ischemic postconditioning

Abstract:Objective To test the hypothesis that mechanical postconditioning with lactic acid and saturated hydrogen saline could mimic the upstream trigger factor of mechanical postconditioning and relieve cardiomyocytes apoptosis through mitochondrion pathway. Methods 108 rats were randomly divided into 6 groups: sham, reperfusion/injury(R/I), mechanical postconditioning (M-Post), lactic acid (Lac,60μl), saturated hydrogen saline (Hyd, 250μl/kg), and lactic acid + saturated hydrogen saline (Lac+Hyd, Lactate acid 60μl+ saturated hydrogen saline 250μl/kg). After 45min of myocardial ischemia, corresponding drugs or mechanical postconditioning were administrated according to groups at the time of reperfusion. After 3 min of reperfusion, PH value of right atrial plasma was measured in all rats. Then the rats were executed at different times. The content of MDA and SOD in ischemic myocardia, the absorbance of mitrochondria with reperfusion after 3 min and 30 min, the expression of Bcl-2/Bax, Caspase-9 Cytochrome c, and cardiomyocytes apoptotic index were detected. Results After 3 min of reperfusion, PH of blood from right atrium in Lac+Hyd group was significantly lower than that in R/I group (7.32 ±0.06 vs. 7.43±0.03,P<0.05); the content of MDA was lower(1.14±0.16 nmol/mgpro vs. 1.56±0.21 nmol/mgpro,P<0.05) and the content of SOD was higher(57.92±15.12 U/mgpro vs. 35.48±12.46 U/mgpro, P<0.05) than those in R/I group. The absorbance of mitrochontria with reperfusion after 3 min and 30 min in Lac+Hyd group were all significantly higher than those in R/I group(P<0.05). The expression level of Bcl-2 in ischemic cardiomyocytes in Lac+Hyd group was significantly higher than R/I group (1.06±0.13 vs. 0.02± 0.01, P<0.05), the level of Bax (0.41±0.06 vs. 2.40±0.45, P<0.05), Caspase-9(0.32±0.08 vs.1.48±0.21, P<0.05) and Cyt-c (0.76±0.09 vs. 6.69±1.26, P<0.05) were all lower than those in R/I group. Apoptotic index of Lac+Hyd group was much lower than that of R/I group (9.50%±1.51% vs. 15.21%±1.91%, P<0.05). At the sametime, all above variables of Lac+Hyd group were not significant different than those of M-Post group(P>0.05). Conclusion Lactic acid and saturated hydrogen asline could mimic the upstream trigger factor of mechanical postconditioning and attenuate cardiomyocytes apoptosis through mitochondrion pathway.
