Clinical effect of rehabilitation exercise therapy in elderly patients with coronary heart disease treated with revascularization therapy

Abstract:Objective To observe the influence of rehabilitation exercises guided by treadmill test on heart function and biochemical indexes in patients with coronary heart disease (CHD) treated with revascularization therapy.Methods The patients (n=100, male 80, female 20 and aged from 60 to 78) were chosen from Mar. 2009 to Mar. 2011, and randomly divided into control group and exercise group (each n=50). The control group was given conventional secondary preventive treatment for CHD, and exercise group was additionally given regular rehabilitation exercises guided by treadmill test for 6 w. The changes of body mass index (BMI), fasting plasma glucose (FPG), total cholesterol (TC), triglyceride (TG), high-density lipoprotein-cholesterol (HDL-C), low-density lipoprotein-cholesterol (LDL-C), N-terminal pro brain natriuretic peptide (NT-proBNP), left ventricular end-diastolic inner diameter (LVEDd), left ventricular ejection fraction (LVEF) and 6-minute walk test (6MWT) were observed and compared before and after treatment.Results After treatment, heart rate (HR) and systolic blood pressure (SBP) decreased and LVEF and 6MWT increased in 2 groups (allP<0.05). After treatment and compared with control group, HR and SBP decrease, and LVEF and 6MWT increased in exercise group [(77.1±10.9) time/minvs. (72.4±8.1) time/min], [(133.0±11.2) mmHgvs. (122.4±14.1) mmHg], [(48.0±2.9)%vs. (56.1±3.3)%], [(204.8±86.4) mvs. (324.5±70.8) m, allP<0.05]. After treatment for 6 w, TC and NT-proBNP were improved in 2 groups (allP<0.05). After treatment and compared with control group, HDL-C increased, NT-proBNP and BMI decreased in exercise group [(0.98±0.29) mmol/Lvs. (1.14±0.33) mmol/L], [(467.8±109.4) pg/mLvs. (423.2± 113.5) pg/mL], [(23.87±1.88) kg/m2vs. (22.83±1.21) kg/m2, allP<0.05].Conclusion The conventional secondary preventive treatment for CHD combining rehabilitation exercises guided by treadmill test can effectively improve heart function and glycolipid metabolism in patients with CHD treated with revascularization therapy.
