Nutrition support for cardiac function and inflammatory factors in patients with chronic heart failure

Abstract:Objective To compare the efficacy of enteral nutrition (EN), parenteral nutrition (PN), combination of parenteral and enteral nutrition (EN+PN) on cardiac function and inflammatory factors in patients with chronic heart failure.Methods 65 patients with chronic heart failure was randomly allocated into three groups: EN+PN group (21 patients), EN group (23 patients), PN group (21 patients). Clinical efficacy was compared after 2 weeks’ treatment. Blood inspection, ultrasonic testing and other methods were used to detect clinical nutrition index, blood lipid, cardiac function and inflammatory factors.Results After treatment, triceps skin fold thickness (TSF), arm muscle circumference (AMC), serum albumin (ALB), pre-albumin (PAB), transferrin (TF) improved significantly in all three groups (P<0.05). Compared with those in EN group and PN group, the rise of TSF, AMC, ALB, PAB, TF in EN+PN group was significant (P<0.05). But no significant differences were found between EN group and PN group. After treatment, cholesterol (CHOL), triglyceride (TG) increased in all three groups than before (P<0.05), but there was no difference between any two groups (P>0.05). Left ventricular ejection fraction (LVEF),amino terminal B-type natriuretic peptide precursor (NT-proBNP) increased; and interleukin-6 (IL-6),tumor necrosis factor α (TNF-α) decreased after treatment in all three groups (P<0.05). The indexes in EN+PN group were lower than other two groups (P<0.05); and there was no different before and after treatment in other two groups (P>0.05).Conclusion Combination of parenteral and enteral nutrition produces much better in short-term effects than EN or PN alone for chronic heart failure patients.
