Clinical significance of kidney injury molecule-1 in early diagnosis of contrast-induced nephropathy

Abstract:Objective To investigate the significance of kidney injury molecule-1 (KIM-1) to reviewing early nephrotoxicity of contrast in patients undergone coronary angiography (CAG) and/or percutaneous coronary intervention (PCI). Methods The patients (n=46) undergone CAG/PCI were chosen from Feb. 2014 to Jun. 2014. The changes of blood urea nitrogen (BUN), serum creatinine (SCr), cystatin C (Cys C), KIM-1, urinary micro albuminuria (U-mAlb) and urinary NAG enzyme were detected before and 12 h and 72 h after CAG/PCI. Results Of 46 patients without kidney injury before CAG/PCI, there were 4 (8.70%) with contrast-induced nephropathy after CAG/PCI. The levels of Cys C, u-mALB and KIM-1 showed significant difference 12 h after CAG/PCI (P<0.05), and showed no significant difference 72 h after CAG/PCI compared with those before CAG/PCI (P>0.05). The levels of Cys C, u-mALB and KIM-1 showed significant difference 72 h after CAG/PCI compared with those 12 h after CAG/PCI (P<0.05). The levels of BUN, SCr and NAG enzyme showed no significant difference 12 h after CAG/PCI compared with those before CAG/PCI (P>0.05), and showed also no significant difference 72 h after CAG/PCI (P>0.05). The levels of NAG enzyme and SCr showed no significant difference, and BUN level showed significant difference 72 h after CAG/PCI compared with those12 h after CAG/PCI (P>0.05). Conclusion KIM-1, Cys C and u-mALB are sensitive indexes reflecting early nephrotoxicity of contrast.
