Long-term changes of left ventricular diastolic function in patients with acute myocardial infarction after ;primary percutaneous coronary intervention

Abstract:Objective To discuss the long-term changes of left ventricular diastolic function (LVDF) in patients with acute ST-segment elevation myocardial infarction (STEMI) after primary percutaneous coronary intervention (pPCI). Methods The patients (n=36) with successful pPCI were chosen continuously from Jan. 2014 to Jun. 2014, and finally 33 were included after followed up for 3 m (male 28, female 5, aged from 37 to 83 and average age=61±14). All of them were given tissue Doppler echocardiogram detection for reviewing left ventricular function 24 h-48 h after pPCI and during 6-m follow-up period. The observation indexes included left ventricular peak early diastolic mitral flow velocity (E), diastolic mitral annular velocity (e’), left ventricular ejection fraction (LVEF) and wall motion score index (WMSI), and left ventricular filling index (E/e’) was calculated. Results During 6-m follow-up period, LVDF grading was improved in 14 cases (42.4%), deteriorated in 5 (15.2%), and had no changes in 14 (42.4%), and the indexes of e’ [(10.5±3.6) cm/s vs. (8.6±2.1) cm/s], E/e’ [(7.3±3.2) vs. (8.8± 3.1)], LVEF [(59.3±4.2)%vs. (56.0 ±5.4)%] and WMSI [(1.31±0.26) vs. (1.52±0.35)] were improved compared with those 24 h-48 h after PCI (all P<0.05). The level of brain natriuretic peptide (BNP, r=0.64, P<0.01) and level of C-reactive protein (CRP, r=0.47, P<0.05) were positively related to E/e’. Conclusion The left ventricular diastolic and systolic function will be further improved after a longer period than that after a shorter period after successful pPCI in patients with acute STEMI.
