Influence of hierarchic management intervention on hypertensive patients in community

Abstract:Objective To observe the intervention effect of hierarchic management on hypertensive patients in community.Methods The hypertensive patients (n=120, male 63, female 57 and aged from 41 to 75) were chosen from Jan. 2013 to Jan. 2014, and randomly divided into observation group given intervention of community hierarchic management and control group not given community intervention (eachn=60). The blood pressure control and curative effect were compared between 2 groups, and corresponding health knowledge was assessed in 2 groups. The health behaviors were surveyed by using questionnaire in observation group.Results Compared with control group, systolic blood pressure [(138.1±16.2) mmHgvs. (127.7±12.0) mmHg] and diastolic blood pressure [(92.3 ±10.8) mmHgvs. (81.3±9.5) mmHg] decreased in observation group after intervention (allP<0.05). The obvious effective rate was 33.3%, effective rate was 40.0% and ineffective rate was 26.7% in control group, and obvious effective rate was 40.0%, effective rate was 56.7% and ineffective rate was 3.3% in observation group. The total effective rate was higher in observation group than that in control group (96.7vs. 73.3%,P<0.05). The proportion of cases mastered well disease prevention knowledge (61.7%vs. 28.3%) was higher, and that of cases mastered (30.0%vs. 36.7%) or that of cases mastered poorly (8.3%vs. 35.0%) were lower in observation group than that in control group (allP<0.05).Conclusion The intervention of community hierarchic management can effectively control blood pressure and improve health behaviors in hypertensive patients.
