Predictive value of fibrillatory wave amplitude on lead V1 of surface electrocardiography to effectiveness of radiofrequency ablation on persistent atrial fibrillation

Abstract:Objective To review the predictive value of fibrillatory wave (f-wave) amplitude on lead V1 of surface electrocardiography (ECG) to effectiveness of radiofrequency ablation on persistent atrial fibrillation (AF). Methods The patients (n=49) were chosen from Mar. 2010 to Dec. 2013 and divided into non-recurrence group (n=33) and recurrence group (n=16) according to results of follow-up, and also divided into atrial flutter (AFL) group (n=10) and non-AFL group (n=39) according to whether AF transferring to AFL after pulmonary vein isolation during radiofrequency ablation. The maximum amplitude and mean amplitude of f-wave on lead V1 of surface ECG were detected and compared among all groups, and predictive value of f-wave to AF recurrence and AFL transferring was reviewed. Results Compared with non-recurrence group, the maximum amplitude [(0.150 ±0.519) mV vs. (0.094±0.480) mV] and mean amplitude [(0.125±0.50) mV vs. (0.077±0.032) mV] of f-wave on V1 lead decreased in recurrence group (all P<0.01). The ROC curves of the maximum amplitude and mean amplitude showed area under curve (AUC) was, respectively, 0.77 (P=0.002) and 0.80 (P=0.001). The cut-off value of the maximum amplitude for predicting recurrence was 0.115 mV and sensitivity was 75.8%and specificity was 68.7%, and mean amplitude was 0.094 mV and sensitivity was 78.8%and specificity was 68.7%. Compared with AFL group, the mean amplitude of f-wave on V1 lead decreased [(0.139±0.052) mV vs. (0.102±0.047) mV] in AFL group (P<0.05). The ROC curves of mean amplitude showed AUC was 0.71 (P<0.05). The cut-off value of the mean amplitude for predicting AFL transforming was 0.109 mV, and sensitivity was 60%and specificity was 61.5%. Conclusion The amplitude of f-wave on V1 lead is correlated to persistent AF recurrence after radiofrequency ablation, and the possibility of recurrence is lower in the patients with higher amplitude.
