Results of carotid ultrasound screening in stroke high risk population

Abstract:Objective To analyze the incidence and severity of carotid atherosclerosis in stroke high risk population by using carotid ultrasound screening.Methods The population (aged over 45) with continuous residence for 20 y were given carotid ultrasound screening in 4 communities of Beijing Dongcheng District and 13 villages of Yangsong Town in Beijing Huairou District by using group random sampling from Jan. 2014 to May 2014. There were totally 1227 cases enclosed (male 588, female 639). All cases were divided into city group and country group according to their living areas, and then given questionnaire, physical examination and carotid ultrasound testing.Results Compared with city group, the percentage of cases with hypertension (57.4%vs. 40.1%), diabetes (31.7%vs. 20.9%), coronary heart disease (24.9%vs. 15.4%) and hyperlipidemia (34.5%vs. 19.6%) decreased, and that of smoking cases (16.1%vs. 31.8%) increased in country group (allP<0.01). In 45-54 group, 55-64 group, 65-74 group and >75 group, the detection rate of thickened intima-media thickness (IMT) was, respectively, 0.10%, 0.17%, 0.34% and 0.35%, detection rate of multiple plaque formation was, respectively, 0.28%, 0.33%, 0.42% and 0.64%, and detection rate of stenosis and/or occlusion was, respectively, 0.01%, 0.07%, 0.08% and 0.37%. The percentages of cases with thickened IMT (61.0%vs. 49.5%) and single or multiple plaque formation (52.0%vs. 32.8%) were higher in city group that those in country group (allP<0.01). The percentage of cases with stenosis and occlusion was higher in city group that those in country group.Conclusion Carotid ultrasound screening, with higher detection rate of carotid atherosclerosis, is convenient and efficient in screening of carotid atherosclerosis in stroke high risk population. The incidence of carotid atherosclerosis is higher in city than in country.
