A Meta-analysis on treatment of heart failure with nicorandil

Abstract:Objective To review the curative effect of nicorandil in the treatment of heart failure. Methods The databases of WanFang Database, CNKI, VIP Database, Cochrane Library and PubMed were retrieved with computer for collecting the randomized controlled trials (RCT) about treatment of heart failure with nicorandil until to July 2013. All data was screened, reviewed and extracted from the RCT by two reviewers independently according to inclusion and exclusion criteria, and then given a Meta-analysis by using RevMan5.1 software. Results There were totally 9 RCT included involving 655 patients, and the results of Meta-analysis showed that nicorandil increased left ventricular ejection fraction (MD=8.63, 95%CI: 7.20-10.07, P<0.00001), increased significantly 6 minutes walking distance (MD=54.29, 95%CI:41.54-67.05, P<0.00001), and decreased significantly left ventricular end-diastolic volume (MD=-7.62, 95%CI:-8.86--6.39, P<0.00001), left ventricular end-systolic volume (MD=-5.92, 95%CI: -7.67--4.17, P<0.0001), pulmonary artery wedge pressure (MD=-5.46, 95%CI:-7.32- -3.59, P<0.00001), and brain natriuretic peptide (MD=-93.11, 95%CI: -150.80- -35.41, P=0.002). Conclusion The present data shows that nicorandil can improve heart function and ameliorate myocardial remodeling. More RCT with higher quality are required for further confirming the curative effect and safety of nicorandil in clinical practice.
