Value of microvolt T-wave alternans for predicting risk in patients with heart failure and without ICD transplantation:a Meta-analysis

Abstract:Objective To review systematically the value of microvolt T-wave alternans (MTWA) for predicting malignant arrhythmia and mortality in the patients with heart failure and without implantable cardioverter defibrillator (ICD) transplantation by applying Meta-analysis method.Methods The prospective studies on the predictive value of abnormal MTWA to the patients during exercise load test were retrieved from Jan. 1990 to Jun. 2013. All data was given heterogeneity test through χ2 statistics combiningI2 statistics, and predictive values of MTWA to the risks of malignant arrhythmia and mortality were reviewed by using fixed-effect model of inverse variance method for obtaining the pooled effective indexes including hazard ratio (HR) and 95%CI.Results There were totally 7 studies included involving 2953 cases, and among them 662 with positive MTWA, 1705 with negative MTWA, 340 with indeterminate MTWA, and 246 with non-negative MTWA (including positive and indeterminate MTWA). Compared with those with negative MTWA, the risks of malignant arrhythmia and mortality were higher in the patients with abnormal MTWA (HR: 4.51, 95%CI: 2.28~8.89,P<0.001).Conclusion MTWA has predictive values to malignant arrhythmia and mortality in the patients with heart failure and without ICD in risk stratification.
