Abstract：Filspari(sparsentan),the world's first dual endothelin angiotensin receptor antagonist(DEARA),has been developed for the treatment of IgA nephropathy(IgAN)and focal segmental glomerulosclerosis(FSGS).In February 2023,sparsentan received accelerated approval in the United States for the reduction of proteinuria in adults with primary IgAN who are at risk of rapid disease progression.Clinical studies have demonstrated that sparsentan significantly reduces proteinuria and preserves kidney function in patients with IgAN.Sparsentan exhibits a favorable safety profile,with an incidence of adverse reactions comparable to that of irbesartan.This article reviews the mechanism of action,pharmacokinetics,clinical studies,and safety of sparsentan,aiming to provide a reference for its rational use in clinical practice.
