Comparison of the efficacy of ferric carboxymaltose with oral iron preparations:a real-world study in the total management of iron deficiency anemia

Abstract:Objective:To compare and analyze the differences of ferric carboxymaltose(FCM)and oral iron preparations in elevating hemoglobin(HGB)and ferritin(SF)levels,as well as the safety.Methods:Single-center,retrospective,real-world study design was adopted.The information of patients with confirmed iron deficiency anemia(IDA)who continuously attended at Pingtan Comprehensive Experimental Area Hospital,from June 2023 to October 2024 were retrospectively collected.A total of 96 patients treated with FCM,and 73 patients treated with iron polysaccharide complex were included,and the effects and safety of different treatments after 4 weeks were compared.Clinical data were analyzed by χ2 test,Fisher's exact test,and multifactorial Logistic regression analysis.Results:The FCM group showed significantly higher HGB elevation[(40.77±18.17)g·L-1 vs(27.82±16.99)g·L-1,P<0.01]and SF elevation[(109.17±159.30)μg·L-1 vs(10.55±32.15)μg·L-1,P<0.01]compared with the oral iron preparations group.Low level of pre-drug HGB levels and intravenous iron treatment were the key factors for HGB elevation.There were no serious adverse reactions in both groups,with dermatologic disorders dominating in the FCM group and gastrointestinal discomfort dominating in the oral iron preparations group.Conclusion:In the real-world single-center context of Pingtan Comprehensive Experimental Area Hospital,FCM demonstrated the advantages of rapid elevation of HGB and SF levels and improvement of patients'quality of life during treatment of IDA,and it was recommended to play an advantageous role in the whole process of IDA patients'management.
