Sparsentan:a non-immunosuppressive drug treating IgA nephropathy

Abstract:On February 17,2023,sparsentan received accelerated approval from the FDA in the USA for reduction of proteinuria in adults with primary IgA nephropathy who are at risk of rapid disease progression,generally a urine protein-to-creatinine ratio(UPCR)≥1.5 g·g-1.This makes it the only approved non-immunosuppressive drug for the treatment of immunoglobulin A kidney disease(IgAN).Sparsentan is a novel non-immunosuppressive,monomolecular,dual endothelin-angiotensin receptor antagonist,which can double block endothelin type A(ETA)and angiotensin type Ⅱ type 1 receptor(AT1),and play a role in protecting glomerular podocytes and reducing proteinuria.Although the US FDA drug label contains two black box warnings against sparsentan,clinical trials have shown that sparsentan is well tolerated with a consistent and manageable safety profile.Furthermore,sparsentan provides significant clinical benefit compared to irbesartan treatment,resulting in a significant reduction in proteinuria and preservation of kidney function.
