A case of immune-related myocarditis caused by cardunizumab in treatment of advanced gastric cancer

Abstract:A 72-year-old male patient with advanced gastric cancer received cardonizumab injection(625 mg ivgtt d 1)combined with oxaliplatin injection(200 mg ivgtt d 1)and capecitabine tablets(1 500 mg in the morning,2 000 mg in the evening,orally taken half an hour after meals,d 1~d 14)with 21 days as a cycle.After two cycles of treatment,the patient was readmitted to prepare for the third cycle of medication.The results of myocardial enzyme examination before the 3rd cycle of treatment were as follows:creatine kinase 2 423.00 U·L-1,creatine kinase isoenzyme 92.20 U·L-1,myoglobin 1 499 ng·mL-1,and troponin T 828 ng·L-1.D-dimer was 11.39 mg·L-1,fibrin degradation products were 38.84 mg·L-1,and no chest tightness or chest pain was reported by the patient,suggesting that elevated myocardial enzyme indicators were mainly related to myocardial damage caused by immuno-therapy.After receiving high-dose glucocorticoid treatment,the patient's myocardial enzyme indicators improved.Immune-related myocarditis caused by cetuximab is not common,but it has a potential explosive risk.When patients experience elevated myocardial enzyme indicators,regardless of whether other clinical symptoms occur,timely treatment should be taken,which has important clinical significance for improving patients'prognosis.
