Investigation of the role of serum glycated albumin and other risk factors in the early kidney disfunction in patients with type 2 diabetes mellitus

Abstract:Objective: To investigate the role of serum glycated albumin ( GA) and other risk factors in the early kidney disfunction in patients with type 2 diabetes mellitus ( T2 DM). Methods: The clinical data of 186 patients with type 2 diabetes mellitus were retrospectively analyzed, whose estimated glomerular filtration rates ( e GFR) were ≥90 m L·min-1 and urinary albumin excretion rates ( UAER) were < 200 μg·min-1. The correlations between disease duration, body mass index ( BMI), systolic pressure, diastolic pressure, GA, glycosylated hemoglobin ( Hb A1 c), fasting blood glucose, total cholesterol ( TCH), triglyceride ( TG), high density lipoprotein ( HDL-C), low density lipoprotein ( LDL-C) and e GFR were analyzed statistically. A binary Logistic regression model was constructed in which early kidney disfunction ( defined as 0 when UAER was < 20 μg·min-1, defined as 1 whenUAER was 20 ~ 200 μg·min-1) was assigned as dependent variable and e GFR clinical index as independent variable. Results: Correlation analysis revealed that GA ( P = 0. 025, r = 0. 164), systolic pressure ( P = 0. 002, r =-0. 225), TCH ( P = 0. 002, r = 0. 226), LDL-C ( P = 0. 000, r = 0. 292), and TG ( P = 0. 002, r = 0. 229) had correlation with e GFR, while Hb A1 c ( P = 0. 417, r = 0. 060), disease duration ( P = 0. 710, r = 0. 027), BMI ( P = 0. 058, r =-0. 139), diastolic pressure ( P = 0. 381, r = 0. 065), fasting blood glucose ( P = 0. 999, r =0. 000), and HDL-C ( P = 0. 869, r =-0. 012) had no correlation with e GFR. Logistic regression analysis showed that GA, TCH, TG and LDL-C were influencing factors of early DN. Conclusion: For patients with early diabetic nephropathy, GA, TCH, TG and LDL-C are risk factors. GA can be used as a monitoring index for blood sugar and diabetic early renal damage.
