Factors affecting the efficacy of clopidogrel and its metabolism

Abstract:As a prodrug,clopidogrel (CLP) was always metabolized by cytochrome oxidase (CYP450) to produce active spices.These active species can efficiently inhibit the ADP-mediated platelet activation and aggregation by specifically and irreversibly binding to the adenosine diphosphate adenosine receptor (P2Y12) on the platelet membrane surface.However,more and more events of clopidogrel low response or no response occurred on clinic,which led to intravascular thrombosis and even some serious cardiovascular and cerebrovascular events,such as acute coronary syndrome.By now,the inhibition of clopidogrel metabolic process was found to be the main reason of these adverse events via clinical studies and genetic factors were found to be the major factor for CLP metabolic process.Thus,in this paper,the genetic factors of CLP metabolism and the interaction between CLP and some drugs were reviewed.
