Effect of intracoronary recombinant human prourokinase administered through targeted perfusion catheter on STEMI patients with primary PCI

Abstract:Objective:To investigate the effect of recombinant human prourokinase given via coronary artery through targeted perfusion on myocardial perfusion and prognosis in patients with ST-segment elevation myocardial infarction (STEMI) undergoing primary percutaneous coronary intervention (PCI).Methods:Totally 108 patients with STEMI were retrospectively analyzed,among whom 64 were treated with recombinant human prourokinase (20 mg) and nitroglycerinin (3 μg· kg-1) via distal segment of target lesion through targeted perfusion catheter by using the infarct-associated artery (IRA) followed by conventional PCI,which was assigned to the observation group,and 44 were treated with tirofiban (0.25 mL· kg-1) and nitroglycerin (3 μg· kg-1) before routine PCI,which was assigned to the control group.The myocardial perfusion,range of myocardial infarction (percentage of ST segment elevation),clinical prognosis (MACCE) and safety outcome (incidence of severe hemorrhage) were compared and observed in the two groups.Results:There was no statistically significant difference in the baseline demographics and clinical characteristics between the two groups (P > 0.05).The levels of epicardial coronary perfusion (cTFC) and myocardial perfusion (TMPG) in the observation group was better than those in the control group (P < 0.001).The incidences of nosocomial MACCE and hemorrhage in the two groups were comparable.Conclusion:Administration of recombinant human prourokinase in the distal targeted perfusion catheter can further improve the myocardial perfusion level of STEMI patients,and does not increase the incidences of MACCE and hemorrhage in hospital compared with targetly applied tirofiban.
