Management of adverse reactions of immune checkpoint blockade

Abstract:Objective:To explore the adverse reactions of immune checkpoint blokade (ICB) and effective management principles.Methods:A systemic review of the latest articles was performed to assess the adverse effects of ICB and managements.Results:ICB could lead to a particular group of adverse effects referred to as immune-related adverse events (irAES),which impact on multiple organ systems.irAES takes place throughout the process of the treatment or after it ended.The management principles of irAES include five pillars:prevention,anticipation,detection,treatment,and monitoring.Early identification and detection is the key to management.Conclusion:Though most irAEs can be controlled or it is reversible by systemic administrations of glucorticoids,some of which can develop life-threatening toxicities.Thus,patients education,early diagnose and close clinical monitor are essential to improve the safety of ICB.
