Therapeutic effect of teprenone on Hp-negative atrophic gastritis

Abstract：Objective:To observe the clinical effect of teprenone for Helicobacter pylori (Hp)-negative patients with atrophic gastritis.Methods:Totally 120 patients with atrophic gastritis were enrolled,who were selected by gastroscopy and pathological examination,and diagnosed as Hp-negative by 13C urea breath test,from April 2015 to July 2016 in Department of Gastroenterology,Navy General Hospital.The patients were randomly divided into 3 groups (n =40 in each group),and continuously treated with teprenone,bismuth (colloidal bismuth pectin capsules) and placebo (starch pills) for 12 weeks.Endoscopic observation,pathological biopsy and the changes in gastric mucosal tissue and microcirculation factors were assessed before treatment (0 week) and 12 weeks after treatments.Results:The efficacy was evaluated by endoscopy at 12 weeks after treatment.The total effective rates of teprenone,bismuth and placebo groups were 85％,75％ and 10％,respectively.No significant difference was found between teprenone and bismuth groups (x2 =1.25,P ＞ 0.05);both bismuth and teprenone groups were superior to placebo group (P ＜ 0.05).At the 12th week,pathological changes in gastric mucosa were evaluated.The total effective rates of teprenone,bismuth and placebo groups were 95.0％,87.5％,7.5％,respectively.No significant statistical difference was found betwenn bismuth and teprenone groups (x2 =0.63,P ＞ 0.05);both bismuth and teprenone groups was significantly better than placebo group (P ＜ 0.05).The changes in gastric mucosal tissue and microcirculation factors were compared at the 12 weeks.Teprenone and bismuth were significantly more effective on these changes than placebo (P ＜ 0.05),while the effect of bismuth was similar to teprenone (P ＞ 0.05).Statistically significant changes were found after treatment with teprenone and bismuth compared to before treatment (P ＜ 0.05),but no significant change was found in placebo group (P ＞ 0.05).Constipation was reported in 3 patients and nausea happened in 2 patients in bismuth group.Headache was reported in 1 patient in teprenone group.Conclusion:Teprenone can repair mucosa,and shows a therapeutic effect on atrophic gastritis with higher safety.
