Comparison of the efficacy and safety of thrombolytic therapy using low dose reteplase and alteplase in patients over 75 years with acute myocardial infarction

Abstract:Objective: To explore efficacy and safety of intravenous thrombolytic therapy using low dose reteplase and alteplase in patients over 75 years with acute myocardial infarction ( AMI). Methods; Fifty-four patients over 75-year-old with AMI were enrolled from 2006 to 2009. Patients were randomly assigned to receive low doses of either reteplase or alteplase therapy. Reteplase was administrated as 2 intravenous injections of 5 Mu at a 30-min interval. The rates of coronary artery recanalization at 1 h and 2 h were compared in 2 groups. The incidence of major adverse cardiac events ( MACE) within 30 days and the bleeding relevant complications were compared in 2 groups as well. Results: The recanalization rate of 1 h was higher in reteplase group than in alteplase group (62.1% vs 36.0% , P <0. 05). There was no difference in 2 h recanalization rate between 2 groups. No significant difference in MACE rate and bleeding complications was documented between 2 groups. Conclusion: Low dose reteplase is effective and safe in the treatment of elder patients over 75 years with AMI. It has superiority in coronary recanalization time in comparison with alteplase.
