Recent advances in coronary heart disease without traditional risk factors

Abstract:Coronary heart disease(CHD)without standard modifiable risk factors represents a high-risk subgroup with poor prognosis that is often underestimated.Despite the absence of traditional risk factors,this population experiences higher short-term mortality and is often undertreated.The pathophysiology of CHD in these individuals transcends conventional frameworks and involves factors such as genetic predisposition,chronic inflammation,and environmental exposures,including elevated Lp(a)and hs-CRP.Clinical identification necessitates the integration of family history,Lp(a),hs-CRP,and coronary CT.Management emphasizes strict standard secondary prevention plus individualized targeted interventions for those with high inflammation or elevated Lp(a).Precision management of this subgroup is a key direction in cardiovascular precision medicine.
